Job Description

We are seeking a skilled and compassionate Community Care Assessor to join our multidisciplinary team at Whiston Hospital for an initial 3 month contract. You will be responsible for assessing the needs of individuals, utilising a strengths-based approach in line with relevant legislation, including the Care Act 2014, Mental Capacity Act (MCA), Continuing Healthcare (CHC), and Discharge to Assess (D2A). The goal is to identify care and support needs, develop person-centred support plans, and ensure positive outcomes for service users.

Key Responsibilities:

Conduct assessments of need within a hospital environment using a strength-based approach, adhering to current legislation.
Determine individuals' care and support needs, ensuring compliance with the Care Act 2014, MCA, CHC, and D2A guidelines.
Design, agree, and manage personalised support plans while effectively managing expectations.
Complete all reviews and reassessments to evaluate positive outcomes, address any barriers to achievement, and adjust care packages as needed.
Provide accurate, timely, and up-to-date data entry on all cases using departmental IT systems and databases.
Offer advice, information, and support to help individuals access universal services and community resources.
Act in a preventative and enabling role for individuals not eligible for services by providing advice, information, and signposting to local initiatives.
Be an active participant in the Multi-Disciplinary Team, collaborating with internal and external partners to ensure cohesive service delivery.
Perform risk assessments for individuals accessing services, including within their home environment, whilst following safety and lone working policies.
Manage and respond to the team duty inbox, including emergency visits and assessments as required.
Promote effective communication with service users, their families, and carers, fostering trust and transparency.
Develop and maintain strong working relationships with health and social care agencies, including NHS, Police, Fire Service, and other key partners.

Requirements:

Experience in a similar role, preferably within a hospital or healthcare setting.
Strong understanding of relevant legislation including the Care Act 2014, MCA, CHC, and D2A.
Proven ability to conduct strength-based assessments and develop person-centred care plans.
Ability to work collaboratively within a Multi-Disciplinary Team.
Excellent communication skills and the ability to build relationships with service users, families, and external partners.
Competence in data entry and use of IT systems for case management.
Strong organisational skills with the ability to manage multiple tasks and respond to urgent requests.
Experience in conducting risk assessments within various settings.
